The Cortes Community Health Association is embarking on a review of our Strategic Plan in order to make sure that we are doing our best to serve our Community now and moving forward into the future.

The first step in this process is Community Engagement and we want to hear from you!

Please look for our table at Cortes Day as and take a moment to fill out our survey. All data collected will remain anonymous and give us valuable information as we plan for the future of our organization.

All participants can enter their name into a draw for a prize to be drawn later in the summer!
